The possessive usually comes with the article: il mio libro, la mia casa. It agrees with the thing owned, not with the owner: Marco e la sua macchina, Anna e il suo cane.
Exception: no article with one family member: mia madre, tuo fratello, suo marito — but i miei genitori (plural), la mia mamma (affectionate form) and always il loro: il loro figlio.
suo means his, her and also your (formal Lei): Ecco il Suo passaporto, signore.
